External internship course intent form
The purposes of this form are to advise the director of internship programs that you have satisfied or soon will satisfy eligibility requirements for the internship program and to indicate that you intend to enroll for an internship in the next semester.
Students must be enrolled in or have completed KINE-P 443 prior to submitting the form for KINE-P 393. If you do not provide all of the necessary information and materials, your application will not be processed. If you have any questions, please email kineint@iu.edu.
